Living Well With Chronic Conditions
Living Well with Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like arthritis, mental health conditions, chronic pain, cancer, or diabetes can cause those affected to lose physical conditioning and suffer health problems over many years. This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with a chronic health condition.

ilities. This could be a laptop or tablet with a camera and microphone or a smart phone. Registered participants will receive registration information prior to the class start date. We will hold a "session 0" where participants can meet one another, practice using Zoom, and receive support for technical challenges. This session 0 will be held on Thursday, January 14 at 1:00PM. Registered participants will receive the Zoom link for the session 0 and each class session via email.
